Welcome to Dune Lakes Elementary School!

New Student Enrollment Checklist

Below is a list of documentation we will need in order to register your child.  Please return the completed packet (Items 1-9) to the Dune Lakes Elementary Front Office. A printable PDF is located in the files below.

  1. Parent picture ID verification

  2. Completed Registration Form (Original Registration Form is available in the Dune Lakes Front Office when completed packet is submitted.)

  3. Copy of original Birth Certificate (Student must be 5 years old on or before September 1st. A Birth Certificate can be obtained for any child born in Florida through the Walton County Public Health Unit for $12 - $18.25 depending on location.

  4. Copy of Social Security Card (requested)

  5. *Florida Health Exam (school physical, front and back) Physical exam cannot be expired.  It must have been performed in the state of Florida, within 1 year of start date in August 2024.

  6. Florida Shot Record on DOH 680 - FL Certification of Immunization (blue card) 

  7. Proof of Residence for Walton County (see requirements below)

  8. Grade placement criteria (If student is transferring from another public school, we can use the records that indicate grade placement.  If transferring from a private school, we will need to screen for placement if state achievement testing results are not available.)  (We will request records from your previous school listed on registration form.) 

  9. Copy of IEP (Individualized Education Plan) if applicable

Revised Proof of Residency Criteria for Walton County, Florida

Below is a list of documentation we will need in order for Proof of Residency to register your child.

Residency 1002.31 F.S. 

Walton County School District requires proof of residency for all students and should be provided by a parent/guardian or adult student. Follow the requirements below that best describes your residency situation.

Note: In all cases a valid driver’s license/ID card, military ID with picture or passport with picture are required for identification purposes

 

IF YOU ARE A HOMEOWNER, you MUST provide the following documents:

  1. Current mortgage statement (dated within 30 days) or deed or property tax record and

  2. ONE current utility bill dated within 30days (for new services, an activation notice may be accepted) or

  3. ONE additional current document showing your address from the list below:

  • Bank statement

  • Credit card statement

  • Insurance policy or statement

  • Paycheck stub

  • Property tax record

  • Vehicle registration

 IF YOU ARE A RENTER, you MUST provide the following documents:

  1. Current lease with the names of everyone living in the household listed on the lease. If the lease is month to month, a notarized letter from the landlord/owner/property manager is required (lease must have both tenant and landlord/property manager’s signature and contract information.) and

  2. ONE current utility bill dated within 30 days (for new services, an activation notice may be accepted) or

  3. ONE additional current document showing your address from the list below:

  • Bank statement

  • Credit card statement

  • Insurance policy or statement

  • Paycheck stub

  • Property tax record

  • Vehicle registration

Bus Route / School Zone

The Dune Lakes Elementary School Bus Zone covers South of Choctawhatchee Bay as far West as Highway 393 then East to the Walton County/Bay County border.
